What is the immunization form?
- The Forsyth Tech immunization form is required to attend any clinical setting. There are no exceptions.
- The entire form must be filled out and signed or stamped by an MD, PA or Family Nurse Practitioner, along with the practice or agency’s name.
- The completed form must be submitted the first week of class.
- TB tests are performed one day and read 48-72 hours later. For the immunization form to be complete, the TB test must be read. If positive, treatment must have been provided. If you have previously had a positive TB test and/or been treated for TB – chest x-ray results will be needed.
- If you cannot find your previous records, you will either receive a titer or the injections. Ask your doctor’s office which would be best for you. Health Departments will not draw titers.
- Immunization documents from providers will be accepted and must be attached to the immunization form itself.
