CHILDCARE PROVIDERS must be licensed by the NC Division of Child Development and Early Education. BEFORE- AND AFTER-SCHOOL CARE must be licensed either as a daycare center with the NC Division of Child Development and Early Education or the NC Department of Public Instruction. If you have children attending different programs, list the same information for the additional programs on the back of this sheet. The Carol L. Danforth Early Childhood Lab School is available on the Main Campus for children 3 - 5 years of age.

Cost of Childcare

For the North Carolina Child Care Grant, you must submit documentation of the amounts below (rate sheet or statement of cost on childcare provider’s letterhead showing a daily rate) to the Office of Student Financial Services.

Child Care Grants
North Carolina Child Care Grant

If I am selected for the North Carolina Community College Child Care Grant, I understand that I am responsible for paying childcare costs, and that I will be reimbursed after receipt of the Childcare Payment Request, as appropriate up to a maximum of $750 per month. Two children per family, age birth to 12 years, are the maximum that I can have covered by this grant. My award will be calculated using the verified childcare daily rate. Childcare expenses will be reimbursed for the periods/days when Forsyth Tech is scheduled to hold classes (excludes holidays, breaks, etc.). Further, I understand that I must be enrolled at least half-time (6 or more credit hours, EXCLUDING on-line coursework), maintain satisfactory academic progress, and otherwise meet all eligibility for federal aid, or my childcare assistance will be discontinued. I understand that the choice of a caregiver is mine.

CCAMPIS Grant

If I am selected for the CCAMPIS Grant, I understand my child care provider must enter into an agreement with the CCAMPIS Grant administrator to accept partial payment from Forsyth Tech. I also understand that it is my responsibility to pay the childcare costs not covered by my CCAMPIS agreement with Forsyth Tech to the facility. Two children (age birth to 5 years) per Forsyth Tech student can be covered by this grant. Childcare expenses will be reimbursed for the periods/days when Forsyth Tech is scheduled to hold classes each semester/term. I must remain in satisfactory academic progress (2.0 or above), and otherwise meet all eligibility for a federal Pell Grant with the exception of a prior bachelor’s degree or my childcare assistance will be discontinued.

I must notify the Office of Student Financial Services immediately if my childcare arrangements change.
