If you have been diagnosed with or have been exposed to COVID-19:
The employee must notify their supervisor as soon as feasible about diagnosis or possible exposure of COVID-19.
- The supervisor should notify COVID-19@forsythtech.edu and include the employee’s name, email and phone number.
