Current EWD students can update their mailing address, email address, and/or telephone numbers via their account at https://my.forsythtech.edu. All name change requests must be submitted in person or in writing (see below).
Former EWD students can request name and/or address changes in the following ways:
- In person at:
- West Campus Customer Service Center
- Grady P. Swisher Center
- Mazie S. Woodruff Center
- Northwest Forsyth Center
- Stokes County Center
- Transportation Technology Center
- In writing via mail to:

Note: Mailed requests must be signed and dated.
- In writing via email to EWDCustomerService@forsythtech.edu from their Forsyth Tech student email account or from another school-issued email address (address must end in .edu for security purposes).
All name change requests must include one of the following documents:
- A marriage license
- Birth certificate
- Court Decree
- Valid driver’s license and a signed social security card with matching names
For name change requests submitted in person, bring original documents to locations above.
For name change requests submitted via mail or email, attach copies of documentation to the request.
