Nursing Assistant I Program
The Nursing Assistant I course at Forsyth Tech is a state-approved program that trains students in
basic nursing skills needed to care for patients in healthcare settings. The course emphasizes safety,
infection control, communication, legal issues, and personal care, among other topics. It includes
classroom instruction, lab work, and clinical practice to prepare students for the North Carolina Nurse
Aide Competency Exam. CPR certification is mandatory for all students. After passing the exam,
students are registered as Certified Nursing Assistants I (CNA I).
Please read all instructions prior to beginning your enrollment.
STEP 1: Successfully Pass the TABE Placement Test (NON-OPTIONAL).
You will be testing in the areas of reading and math. Placement testing is administered
by appointment each Wednesday at 9:30 a.m., 12:30 p.m., and 5:00 p.m. on West
Campus in Room 8.To schedule an initial appointment or a retake, call 336-734-7831.
A 30-day waiting period is required after 2 unsuccessful attempts before a 3rd
retake can be scheduled.
STEP 2: Collect the following immunizations/vaccinations and identification
documentation:
• Tuberculin (TB) Test– Negative results of 2 TB skin tests within the last 12 months
(must be administered at least 7 days apart)
–OR–
Negative results of 1 serum blood test (QuantiFERON, T. Spot (IGRA)) blood draw
within the last 12 months
–OR–
Negative chest x-ray within the past 5 years for students who have a history of positive
skin tests with written evaluation by healthcare provider for signs and symptoms within
the last 12 months if x-ray is over 1 year old
• Annual Flu Shot– for all classes between October and March
• COVID Vaccination
• One form of identification must be an unexpired Driver’s License -or- official
government-issued signature bearing photo ID.
• Social Security Card (first and last names must match above ID exactly)
• Copy of High School Diploma/GED -or- copy of unofficial college transcript.
STEP 3: Create Your Complio Account to upload your documentation
• Visit http://forsythtechcompliance.com/ to set up your account
• Select “Nursing Assistant I Program” and choose “New Student Bundle”
• The fee for this service is $95 for 2 years covers the compliance package,
background check, drug screen and tracking tools.
• A direct helpline and video tutorials are available to assist you in using the site.
• Your background check will be automatically initiated.
• You will receive an email from Complio with instructions and documentation
needed to complete your drug screening appointment at an approved testing site.
STEP 4: Once all your Complio requirements have been completed
• Send an email to nai_naii_medaide@forsythtech.edu
• Use the subject line: “Complio Complete”
After verification, your name will be added to the Nursing Assistant waitlist, and you will
be contacted when a class opening becomes available.

- High School Diploma/GED -or- Related college coursework
- Skills include professionalism, teamwork, customer service, communication, organization, and self-motivation skills.
- Safety is key and all precautions must be taken to prevent the transmission of infectious diseases or injury.
- Required Technical Standards:
- Walking/standing for 4-6 hours
- Bending pushing, pulling, or lifting up to 25 lbs.
- Assisting residents safely from bed to wheelchair, moving in bed and walking.
- Operating mechanical lifts

IMPORTANT: Criminal Background Checks/Drug Screening Disclosure
Healthcare facilities that provide clinical experiences for Forsyth Technical Community College programs
require criminal background checks and drug screenings. The healthcare facility holds the right to deny a
student admission into their facility based upon the information presented in the initial background
check and drug screening process.
If a student is denied access to a clinical facility based on criminal background checks, the student will not be able to progress in the program due to the inability
to meet the clinical objectives.
Acceptance into the Nursing Assistant program also requires successfully passing a drug screening.
ONLY ONE (1) attempt is allowed. If the drug screening is flagged (failed) for any reason, the student
will not be able to progress in the program and will be ineligible for placement on the program
waitlist for a period of six (6) months (from the date of the flagged result) after completing and passing a current drug screen. Specific information about
the findings will NOT be disclosed to the college.
This is a Continuing Education (CE) course based on contact hours. Please be advised that this program has firm absence policies for both the classroom and clinical experiences that allow for very limited absences and lateness. All missed time must be made up within a specific time or you may be withdrawn from the program and will need to repeat the course from the beginning to qualify to sit for the certification exam.
